How to Fill Up the Forms That Lacks Spacing

You've just started filling up the forms for the FSW Canadian Immigration application and found that the space provided in the PDF file is not enough.

Then how do you fit the word "Bachelor of Science in Electronics and Communications Engineering" in the box provided?

At the form Schedule A – Background/Declaration [IMM 5669]item #7, for example, you can see that the boxes are too small for the long words.

You can type the abbreviated words in it but it will be informal and inappropriate if you don't type it completely.  There are tons of Degree names in the world and it would be difficult to trace that abbreviation.

We can do something about this though.

How to Fill Up The Forms If There are No More Space?

Actually, the form stated that the information can be type-written or hand-written.  There are two options that I can suggest.

Option 1 - Type all the words first for the answer of the questions that fits to the boxes and print the whole form, and those that does not fit should be written by hand.

Writing by hand should be in a clear and PRINTED manner so that it can be read clearly by the officer.

Option 2 - Type all words that fits at the other question boxes and type inside the description tab as "Pls. See Attached Sheet".



At a separate sheet of paper, you can create the whole table and indicate your name, title of the document form, and insert all your details.

How to Fill Up The Forms With More Than 5 Entries?
Another problem is that the forms have limited number of rows, especially at the form (Schedule A – Background/Declaration [IMM 5669]item #8 "Personal History").

The only option that I can suggest to you is to type in all the entries that fits to the form.  After the last entry, you will type at the Activity section as "Additional Details at Attached Sheet". It should look like this...


Then you need to create an attachment with your additional details and insert that right next to the form's page.

I made sample attachment sheet for your reference.  Please do take note that all contents on that sample sheet are all fictitious.

    Thank you for this post. Then how about those people who have more than 5 entries? That No 6 entry will just appear in the attached document right? I have that issue of more than 5 entries for the question "Personal History".

    I am unable to type in my work experience "Financial Analyst" complete in the box.

    Should I do the same for that as well in the Economic classes section. Also do you staple the forms together?

  3. Hi, I am an OFW and my work pass expires in August 2016. How do I enter my latest work experience in the Personal History? Should it be 2014-08 to 2016-08 (validity of my work pass)? or should it be 2014-08 to 2014-09 ( 'to date' to be based on the month of my application)? Thanks!

  4. Thanks for the quick reply! I also have the following questions : under the same form, under the parents' details.. how did you write your parents' name.. did you include the middle name under the given name? When I was in high school, I stopped school for a year, also changed school.. should I mention this under the Education section? Thanks again.

    1. Parent's Question (IMM5669, #4,5): only the the given name and family name...NO middle name.

      Put the education levels and school year in details and institution attended. In my opinion regarding the gap of school year, you don't need to provide that gap info since this question is pertaining to your education only.

      For the date gaps, it should be at the Personal History. In your case, it will not be included since it happened when you're under 18.

  5. What should I do with activities overlapped in personal history question?! For example: working part time during full time studying.

    1. I suggest to indicate as follows
      ex: Cook: Full time (Mon-Fri - 8-5pm),
      then another entry job like
      Carpenter Part Time(Mon-Fri 6-8pm).
      If it doesn't fit, attach another sheet.

  6. Hi Ben good day. Im am little bit confused on question #12 (addresses). I lived in Pangasinan,Philippines from january 2004 -february 2014 and now im working here in Newfoundland,Canada since february 2014 with 2years contract. I will send my application this month of December. Is it right that I should place February 2014 - December 2014 (which is the month of my application) or the same with what reflected on my contract which is February 2014 -February 2016. Thanks

      Jan 2004-Feb 2014 -Pangasinan, Philippines
      Feb2014-Dec2014 - Newfoundland, Canada

      Dont' be confused, they are just asking your address to the present and that doesn't include the future. Your address is where you live since 2004 until the date of your application.

    1. how about forms that are not applicable to me such as, add dependants, sched 4a business nominees, sched 5 Quebec and use of representative should i fill it up even though there are not applicable to me? or i just need to cross them out in the checklist and not include them in mailing my application?

    1. For drafting, Yes, for final submission, I recommend that you use a pen. The best is typewritten or computer printed.

    Any idea on how did you manage to put the Birthdate according to their format (yyyy-mm-dd). Cause whenever I click an automated calender opens and the format is by default (dd-mm-yyyy). I tried changing settings from my pc. But it is not working? Do you suggest to put that in ink rather than printing then?

    You are doing great advise. Hats off. I filled out IMM 5669 Personal history like this:

    2015-01 to today
    2014-07 t0 2015-01
    2014-07 to 2014-07 ( worked for 5 days only in one company)
    2014-05 to 2014-07
    2013-11 to 2014-04

    is that looking ok ?. any gaps?. making sense?. let me know your kind opinion.

    I have few questions regarding IMM5669 form:-

    1) Question 2 (Your Full name written in native language) - My native languate is Punjabi so do I need to write my name in Punjabi or only Arabic, Cyrillic, Korean, Japanese are require to mention as example given in the form.

    2) Date of Death:- If death not happened do I need to write NOT APPLICABLE or need to leave date space blank.

    3) Question 7 - i didn't study in trade school do I need to leave it blank or I need to put NA

    Please help

    my wife received aor and i am her dependant applicant. concerning personal history to the question : have you listed all of your military service ? yes / no / not applicable
    i answered not applicable because my military service happened more than 10years ago
    so i didnt have to mention it in the 10years activity part
    will i get rejected for that?

  23. dear ben , My VO Abudhabi has asked me to send schedule A IMM5669 form for the 3rd time and they specified in their email " No gaps allowed in questions 7, 8, 9, 10, 11, 12." this doesn't make sense to me since obviously there is gaps during education so it is impossible to apply their instruction in question 7. can you please help me out , thanks

    While filling the form IMM 5669. I am having problem in mentioning the dates as

    From 2014-03
    To 2016-07 error in 07.

    Error says
    - Personal Histroy-
    The format you entered in the following field was incorrect:
    6. To - MM

    The correct format is:
    Greater than or equal to From(MM)

    How I should write the dates?

    1. HI Arsii, I tried to key in the dates you mentioned and I did not get the errors. Try it again.
      From: 2014-03 To: 2016-07. Note that to type only the numbers "2014-03" and "2016-07" in the box provided.

  25. I have a diploma and a degree. Should I to combine the total number of years of study to answer question 7 in the university/college box or do I simply put on the university duration? My diploma is an ordinary national diploma equivalent to 2 years in the university.

    1. Hi Chichi, put the number of years according to the box provided. For example, the years of getting the degree will be in the "University/College" box, then for the diploma, in the "Trade School and Other post secondary school"

    3. There's a box for the Secondary education that is for the Secondary School diploma. Another box is for Trade School or other Post Secondary School , it's where you can put the college Diploma.

    for the Schedule A form, in the personal history table,
    i am having a confusion there is a gap of 2 months between my diploma course and Engineering Course, this was the period of admission process into engineering college, what should i put for these 2 months in activity column, as i was waiting for my admission to be confirmed into engineering college (usually it takes 2-3 months in India to get admission into engineering college after Under-graduate course)

    For Schedule A, my wife does not have any education past the age of 18? do I leave that section blank including the boxes for elementary school, etc?

    1. No. you can't leave the section blank...indicate the number of years of which she went to school in the box provided. ex. Elementary - 8, High School - 4, University - 0 ....Then in the table, indicate the details of the Scondary/ high school that she had (institution, city, type of certificate, field of study).

  30. Hi ben , in IMM5669 EDUCATION HISTORY it said ( PROVIDE THE NUMBER OF YEARS OF SCHOOL YOU SUCCESSFULY COMPLETED FOR EACH THE FOLLOWING LEVEL . I am completely grad from elementary and High school but in College i only took one semester which is only 6months and i stop i did not pursue my College. So what am i supposed to write on the box ? The option is ENTER ZERO IF NO SCHOOLING AT THAT LEVEL. .hope u cN help me..
